Well close to 10yrs ago I was huge into D&D and real life role playing adventures. I joined an online community on IRC that was a role playing Community where the "world" we played in dated back to the Babylon.
When I joined my username at the time was my name JEllis and my friend from work and the guy who brought me in said it was too modernised to be called "Lord JEllis" so I thought and thought....woke up one morning with the name "Lord Zantheus" and it stuck ever since.
In the past 10yrs I have only eve run into one other Zantheus and it turned out was a guy back from the HEAT days that stole it from me.
Johny Rico is the main character in a book called "Starship Troopers" by Heinlein. I needed a space marine name for BF2142 and Ciaphas Cain was too much like Cain.
Real first name and star wars lover so I added Kenobi just like from the movies

I used to run a few very popular websites for a PC MMORPG, the code name for the project was "Dark Siren", which as an Instant Messaging server and client... I'll spare the details.
I changed games, and my usual online name was taken, so I started thinking, I'd not long put the IM project to rest, and the name was fresh in my mind, so I ended up making up Sirex.
I came to Sirex from my IRC names.
which ended in nex usually,
So i ended up with Sir and ex making Sirex (sigh-rex).
Only issue with Sirex is, people in mmorpgs think ouy ment sir ex not si rex
I've used it for years now, once you hit a certain age I've noticed this the name you use tends to stick with you forever, at least from what i've seen.
I use Sirex and SirexToo (usually the Too one is NEVER taken hehe).
The first MP game I played was Descent. It was a game that involved flying little ship through mine like mazes. You would also try to find a long list of weapons that could be stolen from opponents after you kill. It was a perfect game for a bunch of young flight instructors to play during bad weather. Anyway, I named my guy "The Duke" after John Wane and the problem was it would drop the "E" then people started calling me the Duck and would quack at me when I died. After a while I dropped the "The".
Some friends and I were joking about the idea of starting in a politically motivated band. One of my friends said that the name would have to be a court case or something. And then I suggested that it would need a clever spelling. We came up with Rovey Wade.
Needless to say, we never made the band, but I really enjoyed the name.
I originally started off here at the Lair with seikomusashi. Seiko was a name that randomly came to me one day (Didn't know about the watch company at the time) and musashi was from the game Brave Fencer Musashi. I ended up dropping musashi for the {CLR} tag, eventually dropped the {CLR} tag at one point and never requested it be put back on. Don't really think I need the tag anymore, and i've noticed that people tend to think you're an admin if you have the tag on these days. I'm thinking about a name change though as this one is getting worn out, but it's gonna be one that i'll be sticking with from here on out if I change it.
