Here is what I use for these situations:
Kaspersky Rescue Disk 10
or Bitdefender Rescue Disc
Download either or both ISOs and burn them to CDs. Boot off the CD update signatures and start the scan. Both CDs boot into a live linux OS, great way to insure that nothing of the malware is able to load itself into memory to allow for a total cleaning.
I prefer Kaspersky, but it cant take several hours to do a thorough scan. Had about a 98% success rate in cleaning infected computers depending on how evil the malware was and how well it was rooted in the OS.
