Dust II. It is the most balanced map.

For favorite Source custom maps I always liked Lost Temple best. It to is a well balanced map.

Lost Temple is great!

Honestly I like them all, I like variety and I think most of the maps are in fact balanced, the problem is not the maps it's more the players not playing as a team.

It is mostly teams and a great team can overcome a bad map.
Some maps only have a couple of routes and this tends to funnel players into death zones or stalemates. Dust being the classical example.
